Consumer Health Data Privacy
Washington law requires a separate notice about consumer health data. This is ours. It is short because we collect very little.
Last updated: August 2026. This notice applies to generationsmhw.com and is provided under Washington’s My Health My Data Act (RCW 19.373). It is separate from our Notice of Privacy Practices (which covers your medical record under HIPAA) and our Website Privacy Policy.
Most health information we hold about you as a patient is protected health information governed by HIPAA, and the My Health My Data Act does not apply to it. This notice covers the limited consumer health data that our website may collect.
The short version
- We do not sell consumer health data. We never have and we will not.
- We do not share it with advertisers, data brokers, or social media platforms.
- There are no advertising trackers, no social media pixels, and no Google Analytics on this website.
- Our online self-assessments are scored entirely in your own browser. Your answers are never sent to us unless you deliberately choose to share your score.
What consumer health data we collect
Information you choose to give us. If you request a call back, we collect the name, email address, and phone number you enter, along with anything you write in the message box. If you complete one of our self-assessments and choose to request a call back from the results screen, your score is included only if you leave the sharing checkbox selected. You may clear that checkbox and still request a call back.
Information you enter when booking. Appointment booking is handled inside CharmHealth, our electronic health record. Information you enter there becomes part of your medical record and is protected under HIPAA rather than this notice.
Aggregate website measurement. We use Plausible Analytics to count visits. It sets no cookies, stores no IP addresses, and collects nothing that identifies you personally. It cannot tell us who you are or link your visits together over time.
We do not collect precise location data, biometric data, or health information purchased from any outside source.
Where it comes from
Directly from you, and only when you type it in or press a button that sends it. We do not obtain consumer health data from data brokers, advertising networks, public records, or any other third party.
Why we collect it
To respond to your request, schedule and provide care, meet our legal and professional obligations, and understand in aggregate which parts of this website people find useful. We do not use it for advertising, profiling, or any automated decision about your care.
Who we share it with
We share consumer health data only with service providers who process it on our behalf, under contract, and only as needed to operate the practice:
- CharmHealth — our electronic health record, appointment scheduling, and patient portal.
- FormSubmit — the relay service that delivers call-back requests to our office email.
- Google Workspace — our business email provider.
- Plausible Analytics — aggregate website measurement only; it receives no consumer health data.
We may also disclose information when required by law, or to prevent a serious and imminent threat to someone’s safety. We have no affiliates. We do not share consumer health data with advertisers or data brokers, and we do not sell it to anyone.
Your rights
If you are a Washington resident, you have the right to:
- Confirm whether we are collecting, sharing, or selling your consumer health data, and access that data.
- Get a list of every third party and affiliate with whom we have shared it.
- Withdraw your consent to our collection and sharing of it.
- Have it deleted.
We will not deny you care, charge you a different price, or provide you a different level of service because you exercised any of these rights.
How to make a request
Call us at (206) 923-8886 or email info@… with the subject line “Consumer Health Data Request.” We will respond within 45 days, and will tell you if we need a 45-day extension. We may need to verify your identity before acting on a request.
If we deny your request you may appeal by replying to our response. If your appeal is denied, you may submit a complaint to the Washington State Attorney General at atg.wa.gov/file-complaint.
Please note: deleting consumer health data does not delete your medical record. Records we are required to keep as a licensed healthcare practice are retained under Washington law and HIPAA regardless of a deletion request.
